
Recognising the need for change from reactive, blame-focused incident management to a proactive, learning-driven approach, Spectrum’s Patient Safety Team successfully implemented the Patient Safety Incident Response Framework (PSIRF). This work, led by Spectrum’s Patient Safety Specialist Jacquie Cotterill, focused on embedding a culture of openness, learning, and continuous improvement.
Key initiatives of the framework included the introduction of “10 at 10” meetings; these encourage open conversations around incidents, improve reporting, and support early identification of risks. The meetings also bring together multidisciplinary teams, fostering collaboration and shared responsibility for safety.
Since March 2023, the team has reviewed 10,098 incidents creating an environment of transparency and active staff engagement. Alongside structured governance, regular safety huddles, and system-based learning, these initiatives have strengthened patient outcomes, supported regulatory compliance, and built a robust no-blame environment where staff feel empowered to speak up.
Commenting on the achievement, Jacquie Cotterill said: “Being named a finalist for Team Of the Year at the HSJ Patient Safety Awards is a testament to the hard work and dedication of our whole team. We are proud of how far we have come in embedding PSIRF and creating a culture where safety is everyone’s responsibility.”
