Our Primary Care Services

Primary Care is an important element of our Spectrum service portfolio.

Many of our service users need support in the community with a range of health and social issues. A number of our service users come within the NHS Core20PLUS5, a national NHS framework which identifies those who need the most support in order to reduce healthcare inequalities. Working in community based Primary Care is an important part of our service framework and fits perfectly with our vision and mission. It is also a great way to give something back to local people in the Wakefield District, where our roots began, as part of our social value.

Tieve Tara Medical Practice joined Spectrum in 2021. At the heart of the local community, this gives us an incredible opportunity to work with patients and improve our services by working together. We are proud to be able to build on a legacy of authentic family practice, with a fantastic team dedicated to tackling the root causes of health inequalities. It also enables us to contribute to the local Primary Care Network and play our part in developing personalised, integrated health and social care services.

Our WY-FI Plus service works with some of the most vulnerable people in the Wakefield District. We provide a referral service for adults with multiple and complex needs, including people who are homeless. We support individuals on a 1 to 1 basis in accessing health and community services which might otherwise be just beyond their personal reach. With the right help and support, we encourage those we work with to grow in confidence and engage with their local communities.
